Using Customs Forms on International Mail

Customs forms must be filled out for all parcels going to any foreign country, including those being sent to military addresses. All NIH-generated, internationally bound parcels sent through the U.S. Mail are checked by the Division of Mail Management Services (DMMS) for the presence of completed customs forms.

The United States Postal Service is only accepting computer-generated forms. The forms can be created online at: https://www.usps.com/international/customs-forms.htm. Simply read the instructions and answer the questions to automatically generate a correct, completed form (generates three copies) which can be printed out on your local printer.

Place all three printed copies into the clear envelope (2976-E — available through DMMS customer service) and attach the clear envelope to the back or side of the parcel.
